In order to promote safety, we require all volunteers working with children or teens to complete a background check before serving in these areas. Below, you will find two options for how to complete the background check. You have the option to download and print off the pdf form, and then bring it to the church office or email it to secretary@f1naz.org, or you can fill out the google request form and an online form will be sent to your email.
